Madhymik Shiksha Parishad Uttar Preadesh, Allahabad revised its TET result for some of candidates, And application form required for both Primary / Upper Primary Result.

To avoid problems/ confusion , Best way is to keep update yourself news related to TET / website of UP Edu. Dept. and to contact concerned authority/department.

Make xerox copies of application forms (applied for Primary Teacher) , speed post/registered post , Photocopy of Demand Draft. And all necessary documents.

Some examination (Banks etc. ) is recommend to write your name / roll no. / date of births/ Father's name/ Address etc. when you are applying for exam.

In my personal openion (But you must consult concerned dept. / authority/DIET where you are applying also, As they are appropriate authority in this matter)  -
You can write your name / roll no. / date of births/ Father's name/ Address etc. behind the demand draft.

I felt a declaration form ( please go through advertisement carefully) also required for the details of ORIGINAL DEMAND DRAFT etc.
